I connected the 2nd DIR to the back of the 1st one, ya double NAT but the 2nd one once it was setup, found and said there was a FW update so I used the routers updater to update it. I then disconnected it after a reboot and while my wired PC was connected to the back of the 2nd DIR unit, I configured it for extender mode, after it rebooted to extender mode, I logged in and found that a select wifi SSID window was on the screen, I found the 1st DIR routers wifi, I selected the 5Ghz SSID, asked to combine the SSIDs and applied the change. After it applied and rebooted, it connected. I had service thru the 2nd DIR unit. I then turned OFF the 2nd DIR unit. Connected a LAN cable to the back of the 2nd DIR to the 1st DIR routers LAN port. Powered ON the 2nd DIR and it connected or seemed to connect but I couldn't tell if it was actually wire connected unit I put the 1st DIR router online as my main host router and then connected the 2nd DIR at his remote location to a ethernet switch located there. Once it was online I connected my iphone to the 2nd DIR at the remote location and watch the LED activity on the switch as the test was running. I saw constant activity on the switch while the test was running so I believe the 2nd DIR IS in fact ethernet connected. The Quality status shows AMBER or RED  though on the host routers web page.

You don't need an extra switch to see the activity and confirm the ethernet backhaul. Once you login, click the arrow to see the extenders map.

Click the icon corresponding to the router (main unit) and you'll will see all devices connected to it. Scroll down and you will see the extender listed as "LAN_MeshExtender" - otherwise, it will show "WiFi_5G_MeshExtender".

The map is misleading and should be better done. It should show different lines for different connection types (perhaps a dotted line for WiFi and a straight line for ethernet). Also the line is red which to me means "Low quality" but it's actually connected via ethernet 1 Gbps. This also goes for the icons on the main page, showing the ethernet connection as "weak" when in fact I get close to 700 Mbps on speedtests for devices connected on that node.

I keep checking for FW updates because 1.01 works but D-Link could fix the way they give information on these admin pages.

Ya, I needed to use a switch to actually see if the data was flowing from the B point to the A Point with something connected at the B point which is seems to be as I saw data indicating on the switch as the test was running on the phone. Nothing in the UI gives any indication of data flow wireless vs wired here.

Ya, The UI is not informative or specific enough to tell if the B point is wired or wireless. Colors is mis-leading as well as this would be if the B point was wirelessly connected. Ya, it would be nice to have some dotted lines for a wireless connection and a solid line for wired. Make the color GREEN as well.

v1.01 is the most current.
