Is there a limit of 5 cameras on any given router? I have 6 cameras in my home and only 5 of them work. Is there a limit or do I have a defective camera?

Are you saying only 5 of the 6 camera's show up in mydlink+
If that's the case make sure the sixth camera is linked to your account.

I've received some clarification from D-Link regarding mydlink device limitations:
- Each mydlink account can support up to 99 devices
- 10 viewers may concurrently view each camera (unless the particular camera model has a constraint below 10)
- 3 viewers may concurrently view each camera if UPnP is disabled on the host router and you are using the mydlink services as a relay for viewing the cameras (Note: This configuration will encounter the 1-minute interval limitation)
