DIR-655 WebFilter Problem with hardware B1 Firmware 2.11NA

I have several DIR-655 routers with hardware B1, firmware 2.11NA and have problems getting web filter to block sites.
Setup access in web filter the site URL, like /www.facebook.com, and then create and enable access control rules with "other machines" - "deny some" and check the appropriate boxes under access control to apply "web filters" and "block some access".  What am I missing to prevent workers from accessing face book during working hours?  Is there another setting I need to tweak as well?
I've tried rolling back to firmware 2.00 - and the others you can download from D-Link, but it just doesn't seem to work on any.

 Router Access Control, Blocking, Web Filters and Scheduling FAQ
To set up scheduling:
1. Reserve device IP addresses ON the router under Setup/Networking.
2. Set up a schedule under Tools/Schedule on the routers web page. Customize your dates and times here.
3. Use Advanced/Access Control configured and apply the customized schedule to a policy and to the PC(s) or device(s) desired to be controlled. 
4. Ensure Time is set correctly on the router. Tools/Time.

I do believe Setup/Networking/DNS Relay and or Setup/Internet/Manual/Advanced DNS Services need to be enabled along with configuring Reserved IP addresses, Scheduling, and Access Controls.

We are on a Fiber connection through our phone system now.  I don't know the phone system model being used right off, but it runs all our incoming phone lines with a rotary hunt type setup, and then provides ISP service over fiber ports built into the unit that comes out Ethernet to our routers.  It is all suppose to be static IP's as well. 
Thanks for the other information, I'll check the stuff out, but an initial response from D-Link support...they didn't know right off and wish for me to call them directly.  Perhaps they wish to Bomgar in, or some remote session to investigate.  I have not done that just yet, and was seeing if anyone in the forum has had the same problem as I.  T

And I'm in American Samoa if I didn't, or failed to answer that part of it. :)

From what it appears, there are additional settings required other than just setting up "access control" and "web filter" settings and the rules under "access control" and checking the box "apply web filter" and "for other machines".  The instructions do not mention this stuff.  And hoping to see if anyone else has actually run into this issue with the particular router.  Seems it should work with just those and we should not have to go on a wild goose chase for additional settings outside of those two basic the instructions provide...go figure huh... :)

If someone has this specific router DIR-655 hardware B1 and Firmware 2.11NA, try your router to see if it works as it should.  I'd like to know too :)
